How to add a newborn baby (under 52 weeks or one year) to your Medicare Card

Medicare will add your newborn baby to your Medicare Card once they are enrolled in Medicare.

You should enrol them as soon as possible.

According to Medicare, a newborn baby is up to 52 weeks of age or their first birthday.

After the birth, your hospital or midwife will give you a Parent Pack including a Newborn Child Declaration form. 

There are 3 steps to add a newborn to your Medicare Card:

Step 1:

Make sure your doctor or midwife completes the Proof of Birth declaration on the back page of the Newborn Child Declaration form.

Step 2:

If you have a Centrelink Customer Reference Number and Medicare Card, add these numbers to the back page of the Newborn Child Declaration form.

Step 3:

Send the form to Medicare:


How to add a baby (over 52 weeks) to your Medicare Card

If your child is over 52 weeks old, you need your baby’s birth certificate to enrol them in Medicare.

How to add someone under the age of 15 to your Medicare Card

If you are a parent or guardian, you can add a child under 15 to your Medicare Card.

  • Birth Certificate showing parental responsibility.
